Starting a reading and math tutor program for K - 12 grade

I am starting a reading and math tutor program for K - 12 grade students at my church. I am looking for a print-based program that will allow me to: 1)assess students in math and reading, 2)offer the basic components of reading instruction (phonemic awareness, phonics, vocabulary development, reading fluency, reading comprehension strategies; 3)offer basic components of math instruction (addition/subtraction, measurement, geometry, operations w/numbers, algebraic thinking, fractions, decimals, persents, integers, pre-algebra; 4)ability to track student progress. Jennifer
