Sufferings can be caused by other people. So can sufferings be caused by yourself if you don't well handle things that are supposed to. Take a good health for example. If you don't have proper exercise after long hours' work, you will suffer from something to your health etc...

Mostly, i would attribute sufferings to the no. 2 that you have stated. We suffer the consequences of our deeds and sometimes we donot even have the ability to recognise it. It's kind of self denial. As we sow , so we reap - it's as simple as that. Now, this could be indirectly related to no. 3. Many a times we force others or their actions and thought processes to take a turn that we would want.
Lastly, i would go for Providence or the undefined force on which we have little or no control.

For my opinion, it is a self made. Because it is us who wants to suffer, if we don't want to suffer, we will escape from any situation like that.Partially , we can avoid some people committed mistakes and we have to suffer as a cause, that is unfair advantage, but it is also i guess one source of suffering.
