Vanity mirrors please help.

Does anybody know if there is a "standard" mounting height for vanity mirrors? Also, once I've got the mirror height figured out, I'm going to install lights next to the mirror. In this type of installation are the tops of the lights usually even with the top of the mirror, above, below? (I'll post some pictures of the lights once I find the camera...)
