Death of Winifred O'Leary 1898 to 1900

Young Winifred Maloney of Adrian Pa., married Thomas O'Leary. She had 3 children, and died in her last pregnancy with twins. The twins lived about a week, and Winifred's grave in Anita Cemetary, Jefferson County, Pa., was re-opened, and the twins were interred on top of her coffin. I have been unable to find any death record for young Winifred, my great grandmother. Her husband Thomas abandoned the 3 little living children, and I cannot find him at all in 1900. I found all 3 children living with sisters of Winifred, which were Maloney girls. The children were John T. O'Leary, Marie O'Leary, and Katherine O'Leary. I am very much at a wall in finding Winifred's death, or any information about Thomas. I found him in 1910 with a new wife, Margaret Lenhart, and 3 new children. Any help would be appreciated.
