Most emails come but some don't

I always end up every few weeks going back to about 3 pages' worth of discussion I've responded to. Why? I want to check that I read all of the replies people left to my comments and while I would say almost 95% I did receive via mail, there are a few I didn't even know existed. It's kind of annoying knowing that there's a possibility a comment won't be notified and someone might think you're ignoring them.
