When taking an oath in court has anyone ever said no? And what would happen if they did?

In my country I am pretty sure that you have the choice of swearing on a Bible or stating an oath. If you choose to do neither then you will be considered to be either in contempt of court or you may be asked to step down and your testimony will not be admissible. I think it all comes down to the importance of your involvment as to which situation will apply.

Well to my understanding they have different Bible versions for different Christian faiths; the Koran for those of the Islamic faith and so forth. For those that have no religious leanings at all there is an oath they have to recite.....
