I thought that for certain types they went through the nose, but others they opened up their skull. I have never heard of the eye socket thing lol

well he is an article about a guy who had one, didn't know it and then researched what it was. It is a decent read http://www.npr.org/templates/story/story.php?storyId=5014080 They don't take anything out when they do a transorbital lobotomy.

I found this information about lobotomy. It sounds pretty scary. There are nerves that connect the frontal lobes to the rest of the brain. The idea behind psychosurgery, later proven to be invalid, was that these nerves were somehow malformed or damaged, and if they were severed they might regenerate into new, healthy connections. Contrary to popular conception, the operation was not used only on psychiatric patients. Many people were lobotomized for “intractable pain”, such as chronic, severe backaches or agonizing headaches. © ronaldinu 2008

They go from the top as far as I've heard and witnessed after the fact. Cut the top of the cranium and do their thing then replace and wait for mending of bones.
