The letter is written to the previous school principal asking for them to give the Birth Certrificate of my sister as we are sending her to a new school since September,2009 This is the letter I have written: Respected Sir/Madam, Request of Birth Certificate I, the undersigned is the parent of (child name) who (School Name) till Grade 6, herewith request you to issue her Birth Certificate as we have the need to submitting her Certificate at the new school in which I have admitted my daughter,(Child name). Please do the needfil and oblige. Yours Sincerely, .................. Parent Signature Correct the letter if you find it informal...

Try this Your name: Your address Date: Dear Sir/Madam, May I request you to kindly provide me with the birth certificate of my daughter (name) who was studying in your school in (Grade 6/standard 6)as this is required by her new school where she will be admitted. Thanking you, Yours faithfully,

HI sindu.. I this this is fine ..., if you want the3n i can give you one more format... you can also try this. To, The Principal, -------------, ------------- -------------. Respected Sir / Madam, Subject: Application to return the Original Birth Certificate of ( Child name ). I ( parent name ) mother of your student ( child name ) Who was studying in your institution for the last academic year in sixth standard humbly request you to provide the Original Birth Certificate which was submitted during the admissions of my daughter to you school. As There is a need of that original birth certificate in the new school where is going to be joined. So, kindly do the needful and oblige. Thanking you. Yours faithfully (parent name)

Ita A bit formal! Try this Dear Sir/Madam Request of Birth Certificate I, the undersigned, am the parent of (Childs name) who is in your school, (School name) till grade 6. I request you release her Birht Certificate to me so I can forward this to her/his new school. We look forward to your reply and the birth certifcate. Yours Sincerely
