Most people respond that they are fine, because they realize that the question is usually just a thing that people say when they see each other, and they actually don't care how you are. I'm sure we've all known people that if you ask them that question they'll start in on their list of ailments. We all tend to avoid these people, though.

Some people say "I am fine" other people say fine, thanks. another group people say same word how are you dear, I am fine. I have also seen people saying good morning instead of saying fine. It defers person to person.
