what,s really in their mind?,I can hardly think about it. Their mental and physical health and life span. Maybe they have their own special abilities too,to consider not yet discovered at any point.I wonder if ever they fall in love. They are fun-loving, and their interactions generally involve teasing and making jokes. I actually enjoy their companion.

They look like they're thinking about what mischief they can get into. :) People with special needs (or whatever the politically correct term is nowadays), as far as my experience is concerned, have much the same thoughts on their mind as the rest of us. They may think about their state of health, lifespan, etc. or not, just the same as anyone else. I think everyone has special abilities. My youngest son is autistic. His special ability is acting and making people laugh. My partner has a long list of physical and mental issues, he's still the most loving man I've ever known. :)

I think they are not thinking but enjoying the time. With thoughts in mind, smile looks made and not natural.
