Did you lose weight for good? Any tips?

Hi. I am in my 30' now and just about all my life I tried to lose weight. It's sad really, because when I started I was about 20 kilos ligter than now. How fat was that? I found a program that works well for me and I lost some weight. My trouble is I can't stick with anything. Even though it works, circumstances will push me where I will miss a day or two (or more) and will find it very hard to get back on track. I know I need to change but I just don't know how. Did you change your eating habits and maybe started to live a better life with more exercise after being quite a couch potatoe? Please advice. Help!
