Membership form ideas

Hmm, seems as though I must educate the ill-informed. When a person desires to join our church, a membership application is completed. This gives us necessary information such as; name, address, birthdate, phone number, previous church attended, areas of ministry served in previous church, etc. This also allows us to find out how the person is joining; either by letter, (from their previous church), on christian experience or as a candidate for baptism. This form is used to enter applicant into our database during the regular work week. It also is used to track the completion of classes (4), needed before the right hand of fellowship is given.
