OR
The elephant can sit down and the man can rope the scratcher to a strong stick (making it longer) and scratch the elephants back while it is sitting! =D

| |  Hmm...get a ladder and climb up to the elephant. Or even better yet, find a tree and hang down from the branch and scratch away.
Find a pogo stick and bounce your way to the top. Or find a whole in the ground and have the elephant climb in.
