If someone has you 'down to a t' , what does that actually mean?

Basically I just read something an online friend of mine wrote and she had me, 'down to a t', which basically means she got it completely right, hit the nail on the head, that kind of thing.
What I'd like to know is, what does 'down to a t' mean and where did that phrase originate?

Well, I think it's Larry the Lamb, but that's because I don't actually know who he is. Other than that I can't think of anyone else, maybe Larry Grayson as he was always happy and gay.
