Do you get things done during the day?

We all have priorities and things to do everyday. Before going to bed we think about the things and itinerary that we should have and do the following day. My question is, are you able to follow through all of the things that you think and plan ahead? In my case, I used to but there are some times that I don't. I've got this procrastination problem and I'm striving and doing my very best to get rid of it. Right now, things are getting better as I'm already able to finish many things during the day, productive things that are a part of my priority. Though I'm still on the track of finally getting rid of procrastination, things are are way better now than before. I used to put things tomorrow before to the point that I no longer find it's importance and put it off for good ending up with nothing and unproductive in the process. I now keep an organizer and list all the things that I should be doing the next day, week, and even month. In that way, I can motivate myself to really finish the job and that at the same time the probably of forgetting things decreases. How about you? Do you get the things that you're supposed to do done? Are you dealing with procrastination or other factors that deter finishing such goals/things/task?
