Health Care Proxy Designation

In the hospital setting, when a patient is obviously unable to make decisions on his own, a health care proxy needs to be designated when there is no living will. The next of kin is usually approached first with the request to accept the designation as health care proxy for the patient. A form is then signed by the health care proxy designee and witnessed by two people. Are there any restrictions (i.e., not a blood relative) in who may witness such a form?
