Have you regretted not acting quickly enough?

I once saw a puppy run on the road and a motorcycle coming that I knew would hit the puppy, but I did not react fast enough and the puppy was hit and died. To this day I regret that I did not shout out to warn either of them. Are any incidence where you could have helped but did not ? Do you regret that ?
