I need to make a maintenance plan for the crawler and hydraulic cranes

My company has 11 crawler cranes and 8 hydraulic cranes. Usually the company is too busy and it is very dificult to carry out one good planned maintenance. Anyway, as a firt step, I want you to help me to make a form for the planning. Have you any general form for that?
