How is vacation time determined where you work?

We are open 24 hours a day, 7 days a week, 365 days a year. We do not get any holidays off unless it falls on our regular day off and then sometimes, they ask us to work then too. We get what they call PTO (paid time off). It averages about 2.2 hours a week. We have to put in for it at least 2 weeks in advance. How does it work at your job? Do you have a set vacation time when the company shuts down?
