Public toilet: Can you stand it?

I often find dirty public toilet so that I tried to hold up until I find a comfortable place to do it. Can't imagine myself using a dirty toilet seat.
I often hold up myself from peeing which I know is not healthy because it can create kidney stone.
I can't even really trust a clean toilet seat. I don't understand why people don't care about clean and healthy public toilet. Can you stand a dirty toilet public? What do you do when you want to pee but the public toilet is dirty?

If this is a true phobia,which for many people it is,why not wear a pair of plastic pants over your regular underwear,this way,you can release the pressure and wait till you can get to were there is a clean toilet?
This may sounds strange to most,but if you think about it,it does work,and many do use this method,you just don't see it.

As much as i can hold up too i will til i can find clean toilet. Sometimes, i go inside certain fastfood chain just to have a toilet coz i know they are keeping it clean, then order some snack afterwards. In some case, if i really can't hold it, i use public toilet if there are no other choice, i use plenty of tissue for the surface of the sit.
