Keeping a Wellness Program Journal

For all those people out there wanting to improve their lives. When we were in Health Class we were taught how to make our own personal Wellness program journals to help us make the healthy change needed that is necessary, to improve our quality of life. This can be adapted into any type of self improvement. - DNatureofDTrain First you Get a Journal, and You call it your Wellness Program Journal... This belong is a simple outline to start... with... and write this down.. Start Date ________ End Date_________ and if you are working with a doctor or a therapist, or have a friend who is helping you..Which is actually a good idea, to do a wellness journal as a group project for the support factor. Due Date__________ Then You Write: Primary Goal _________ and then Long Range _________ and Short Range Goals____________ At least three Short ranges goals are good to help make your long range goal. At the end of Each Goal Write Your Dates,, and your Log Checks... Writing about your progress or lack of progress. And Plan a Good rewards for after each goal is reached.. So Write Reward 1.______ 2.____ and 3.________ Then Write about your built in support... Who will help you with the program? ___________ What will they do for, or with you? ___________ When you are doing you goal settings be sure that you are making reasonable goals...... and not setting unreasonable goals that no matter how hard you tried they would not be reachable. Keep a Daily Log of your program, and extra notes of how the program is either helping or hindering you.. But keep with the program to see if there is any good results. Be sure to include in your log and notes.. What has happened in your program - Physically, mentally, spiritually, etc.. What if anything has changed.. Are you enjoying the program or disliking it? Are other people noticing changes? Are they positive or negative? Is your plan of support helping you? How have they helped? And add any other information that is relevant to the wellness program, you feel that needs to be shared in your programs attempts. Make sure that after your daily logs, you include a weekly log once a week... and then see if you have met up to your goals to have recieved or not recieve that reward. Then after your weekly program is over.. This is when You do an evaluation.. and ask questions like these.. What has been my progress and changes? Did I reach my final goal? Why or why not? (I you met your goal -- will you set another one?) Same area? Will you continue on the program? Do you see benefits for making changes in this way? If you did not meet the goal... Did you set the goal to high? Did you work hard enough? Will you try again? How do you feel physically? Mentally? Spiritually? etc..? Have other people noticed changes? This is when you write your honest opinion of the program and the efforts involved in it. This is all part of the evalutation... Then when I was in school This is how they graded this. Total points of the entire project was 125 points. 25 points for Well program sheet. Each log check was 25 points and the Evalutation of the program was 25 points. I was a slacker and refused to do it at the time.. But after the assignment was over I put it into daily action to help me live up to my values better, and I did so well that I was living by a long list of a really strict code. .. I may start to making my own well programs again.. To help me make it to my goals If you want to make a print out... for the weekly summarys You head it Weekly Summary -- Week of: (then leave it blank for date) Then start a new line, and write Date Then leave space to write, and Write Date two more times leaving space for your log behind each one.... Then You write the End of the week Summary and Date. This program can be adapted and used from anything to Quitting Smoking, To Weight loss or gain, To learning or studying, To commiting to memory, to getting used to an actual, to buidling positive habits or dropping negative habits.
