I'm no doctor but my opinion is that they are born that way. I've seen shows where they say from a young age that they felt like they were in the wrong body. I've even seen a show with a seven year old boy whose parents let him dress like a girl. He feels he is a girl.

Everyone has their own idea about this and I don't think any professional has actually come up with an answer. Personally I think they are born that way but who knows!
