Contract for a maid or nanny?

Hello people. I would just like to ask if anyone of you is making a contract everytime you hire a new maid or nanny. If so, could you please give me some tips or pointers on what to include in the contract? I have already some rough ideas but I still want to see what you have or may offer so that I may incorporate them in mine. Thanks!
