Writing a query letter

Has anyone here sent a query letter to a publisher? I have seen some sites online that say you need to practice writing query letters, so that when you want to submit articles to newspapers and such, you know how to present yourself well. What I'd like to do, is what all is included in the query letter? How do you write it? Anybody have any advice?
