what is the biggest technical blunder that you done?

hi yesterday i connected a 110V(input voltage) Adaptor to 220V supply without readng and only realised when the adaptor went kapoot.it was really shameful on my part,being a marine engineer and handled many huge and sophisticated machinery onboard.

I change the photopaper without changing the adaptor, so what happen is that machine always signal an error and it took me 30 minutes to find out why?

The computer lab back in my 2nd year high school can't accommodate all of us in our class. And so me and my buddy didn't have computers, but we don't need it anyway because we're already finished with our exercises. There was this computer that wasn't plugged and so I told my buddy to plug it and turn it on. It turns out that it was a busted computer. After a few minutes of turning it on a smoke came out of it and my buddy was scolded for turning it on. hehe
