We currently only have the following quotas open: Men 65+ years of age who have taken pain medication within the past 6 months (Kadian, Avinza, Oxycontin, MS Contin, Duragesic patch, Opana ER or Oramorph SR) and live in the following states Connecticut, Maine, Massachusetts, New Hampshire, Rhode Island, Vermont, Iowa, Kansas, Minnesota Missouri, Nebraska, North Dakota, South Dakota, Arizona, Colorado, Idaho, New Mexico, Montana, Utah, Nevada or Wyoming.
