What method of birth control do you or your partner use?

Why do you use this particular method and do you have any concerns of it's effectiveness?
I want to know a little more about the options out there from people personally and how well they like them rather just reading about it online. There are many of different methods so hopefully I'll get a few different answers.
I'm currently on the Yaz pill.

Hysterectomy.

Well I have the birth control pill but I have 2 kids so you can see how I failed to take it in the past but lately I have been 100% faithful to these pills.

Yeah, that's the thing that scares me the most. I've forgotten to take it a few times but other than that I've been good.
