Deaf people learn to read lips, so they must develop some sort of language within their head. It's an interesting question, though. I'd love to know the answer of how this works.

People who are deaf from birth are unable to imagine sounds, in the same way that you can't imagine what seeing infrared or ultraviolet would be like. However, people who lost their hearing later in life - even if they were young at the time - will have some memories of sound, and the brain pathways and memories associated with them. They may even dream with sound. Deaf people can "hear" their thoughts, but will "hear" them as sign language, as a sequence of mental gestures.
