Put it in statements directed at you, not at the boss. "I'm unclear on ______ could you please explain to me more about ________? OR "Just to varify. Do you mean _______? If not, could you please clarify for me?

Hi Mridula123, not sure if i understand your question correctly. You just want to question your boss the answer for his decision or question your boss to take/accept your ideas. If is the the fist situation, just question with respect as this normally would not offense him/her because most of the time subordinate might not understand clearly what boss wants. If is second situation, then you would need to have lots of proof and support documents and notes as boss might base on experience most of the time, so to get he/she accept new ideas, it must with fact and result. So all the best for your situation.

hello! it really takes a lot of diplomacy and guts for you to be able to reach out to a boss and perhaps get him to realize some things that needed some correction or whatever without offending hin as you said. If your boss is the type who listens to employees, then it would'nt be difficult for you. But if he's not, you could try getting his attention by being frank but respectful, candid and still courteous... have a nice try ok? and good luck!
