Peanut butter contamination?

Okay so I don't have a lot of time to watch the news but I would like to think that I am up to date on most subjects. Now this one definately through me for a curve ball. I just heard on the hews this morning that there has been a Salmonella contamination of peanut butter. I just can't believe it, is nothing safe anymore? How does this happen? Are the areas where these food are prepared not clean enough? What are your thoughts? Do you think that there is any way to avoid this? What do you suggest the food producers do to prevent future contamination?
