Can a smoker and a nonsmoker have a healthy relationship?

Of couse a non smoker and a smoker can have a healthy relationship. It maybe unheathly to smoke but that does not stop anyone having a relationship.

That depends on how much each person in the couple respects each other. If there is a lack of respect, then no.
