Although 'hot topics' and 'top discussions' are 2 separate menu items on MyLot I can't understand the difference between the two at all. Both literally mean the same - just phrased differently. How is it decided which discussions make it to these two categories?

good question.
i guess top discussions have the most responses
and hot topics have the most views?

I think they separate it by highly rated vs not so highly rated. Of course the highly rated ones being those that go to the top discussions. The not so highly rated ones being those that go to the hot topics. Either that or hot topics are those that aren't necessarily rated high but have the most responses in them.

i think it is that hot topics are topics that the moderator of that day decides is 'hot' so if the moderator happens to hate the idea of people leaving babies in hot cars, she will put most of the topics on child endangerment in the hot topics.
With top discussion it is the one that the myLotters find most important. So if more mylotters read a title such as "My husband wants my mother to go into a nursing home", that will probably find its way onto top discussions, but not the hot topics if the moderator of the day decides something more interesting.
