Hey,
Well I think that the main reason is that the even is a happy one, they can't put a word like 'sad' or something like that, because it is meant to be a joyful moment. And besides, the song sounds good with the word happy in it. Make sure you have a good day, God bless and Happy Lotting!!

Yes, I always put 'happy' before birthday when I wish somebody. I think it is meaningful to use it because I wish them to be happy on their birthday. There may be many problems in life, but birthday is one day I wish all to forget all those issues and be happy. So, I think it is fair to wish 'happy birthday' when I really mean it.

Hmmm...Can we greet someone on his birthday "Sad Birthday to you!" :-O Seems awkward really. But as for me, I always wanna be greeted with a "Happy Birthday" on my birthday even if I feel gloomy or sad. It kinda perk up everything to me knowing there are people who remembered your natal day.
I think the 'Happy' in the greeting is a sort of an accord to wish someone well. Those well wishers of course hope you to be happy on your b-day. :-)

We put happy to wish the person happiness on his/her birthday. We can also say blessed birthday or other adjective but happy is more pleasing to the ears that is why it is often used.
