i think i would say, "God thank you for giving me a chance to live and be your humble servant, forgive me for all my sins, I am glad i'll be giving you back my life. and to my family and friends and love ones..i love you so much and thank you for making my life wonderful.

If I have only I would say to my love ones "I love you so much" and A will say a simple prayer like "Almighty God please forgive me from all my sins and may you welcome me into your kingdom".
