Anotated Bibliography?? Ugh..Help please.

So I am writing a research paper for my writing class and we need to have an annotated bibliography. I have never done one before and I'm not exactly sure of the format and all that. Does anyone know or know of a site that could show me the format and what information it's supposed to have? Thanks.
