Is it true smoke and direct heat of charcoal can causes of cancer?

Hi all, Does anyone know the better way to make grilled fish, I'm using charcoal with high temperature. coconut oil, salt and garlic (and a few spoon spicy sauce) to grilled the fish. But my friend told to me if the fish is too close to the live coal, smoke and direct heat from live coal can causes of cancer, is that true? And if anyone have another way, please tell me how.
