incurable diseases

Inspite of all the progress made in the medical field there are diseases that cannot be cured. Some dieases though have remedies , sometimes go out of control and the patient dies eg.pneumonia. There is a condition known as pulmonery artery hypertension for which no known sure remedy is available. This is a heart/lung disease. Can some one please throw more light on the remedies available? I am not t medical person.
