Your brother, I think, is not adult and seems to be just a child. I am asking this because there are lots of causes of urinay incontinence like difficulties in waking up from sleep, deficient development of the central nervous system, urinary tract infection, abnormality in the urethral valves in boys or in the ureter in girls or boys, lack of secretion of anti-diuertic hormone, diabetes mellitus etc. But in case of children, the cause might be due to a problem known as Primary nocturnal enuresis (PNE). PNE is defined as the condition when a child can control his bladder during the day but does not remain dry at night for atleast a six month period. It is the most common form of bed wetting. Most of the sufferers are boys & the condition may even persist into adulthood. Many parents feel it to be the result of a defect in their parenting. But PNE has nothing to do with emotional stress, poor self-esteem or emotional instability, rather it is a common developmental phenomenon related to physical and physiologic factors. Most children eventually get rid of this problem when they learn to control their bladder. So the parents need not worry, especially if the child is young & the bed wetting does not occur every night, all that is needed is a supportive attitude from the parents and the other members of the family. But if the child is older and frequently urinates at night, parents have every right to be worried and they should immediately consult with a doctor for advice and act accordingly. If the bed wetting is not caused by any medical problem, the following measures may be taken- 1) As bed wetting is beyond the control of the child, the child should not be scolded, rather the parents should be more supportive and treat the problem with sympathy which will help the child to cope with the problem. 2)Bladder training- There would be no restriction of fluid intake during the day but there would be restriction of fluid intake near bed time. Once a day, at the same time each day, the child should be asked to hold the urine as long as possible, at least to the point of discomfort. This will help to gradually increase the bladder capacity during the day. At least once each day the child should practice starting and stopping the stream of urine several times. 3)Responsibility training- The child should be entrusted with responsibilities that suit the age. If the child is old enough, he or she should be taught to change the wet clothes by own efforts after such an act. 4)Lifting-The practice of rousing the child in the middle of night for urination may be helpful. But it gives results only if the child urinates in bed once each night at a regular schedule, otherwise it may hamper the child's progress.

Maybe you should get your younger brother to urinate before bed.
