how do you know if he / she reallly cares?

I just had this question in my mind.. sometimes no matter how someone loves us, or how we love them... we always feel like they are neglecting us (one way or another?)
With your partner... have you ever sometimes felt that he/she is not caring enough for you? Like you have this problem, situation and you do not feel he is ever worried about you.. You clearly tell him your issues and yet he never bothers to even reach out and help you?
Is there really a love in this kind of relationship - or if there is, what kind of love is that - it tends to shut off when there are problems!?

My situation is a little different....I know that my ex husband loves me...But for a time there when we had our huge falling out I didn't think he did because he was so closed off and had alot going on in his head that even tho he was thinking of me and concerned in what I was going through he didn't show it...Well we are currently trying to work things out and get back together and he kind of sluffed me off the other night but he realized what he did and he was concerned...
