I missed the biometrics checkup last week

I should blame on myself, because I missed the biometrics screening check up last week, and it is the only appointment that was available at my work. Otherwise, I have to either go to Walgreen take care clinic, or my home doctor to get the checkup, which I am not exactly sure whether they will charge me money or not. There was a letter sent to me, and I didn't read it thoroughly and toss it away. Until my co - worker told me that I have to sign up online to make the appointment, and they will give me $500 discount on my insurance premium on 2012. What an idiot I am. Now, I have to call the headquarter HR office to get the form and visit one of those clinic myself. Something to make me feel better, there were a couple of co - workers also didn't make it to the appointment last week. So, I guess that I am not the only one.
