What is the most compelling way to ask for a raise?

I want to know the most effective way to walk in my supervisor's office and make the case for a raise. 1. Make an appointment? Tell him why I want to see him? Cold turkey? 2. Bring data and evidence of my productivity/value to the company -- or charm alone? 3. Press him/her for an answer right away, or not? 4. Take him/her out to lunch? Develop a rapport or make if all business? 5. Flirt? Wear a low-cut and bend over the desk? Sleep with him? (does that work?) 6. How often do I go back if I fail? 7. Do you threaten to move on to another company? What's a sure-fire way to make it your best effort with the highest probability of success.
