LOL
Just tell the person to rub some dirt in it! It will be fine!
Just kidding.
We ask because it may not hurt that much. Or we may not realize it was that bad.
My daughter tripped during Trick or treating this year. And of course, I asked if she was okay even though she was crying. Reason being is, she could have just told me it wasn't that bad.
I will do the same thing with my baby..ok, not a baby cause he is 5. But he is my baby...Anyways, I will ask him if he is ok and he will realize it isn't that bad.

Well i think its because the pain have different levels .
So asking the question : Are you alright ? is in a way first to ask if he really is alright and second if we see he is not , just not to bother him if he does not want to be asked . Its simple to answer if you are not wanting to telling someone with just : I'm fine or lie : Ye i'm alright . But still the pain can be enough to bear for oneself so he might feel it but not wanting to share it , because its feeling its not important enough .
Well thats the things i kind of imagine are there in to the question ?
To a lot of questions you might find strange parts :)

I only ask that if they trip, fall or bang themselves on something. You really don't know if they are hurt unless they tell you. If it is noticable that the person is injured I will ask other questions to see if they are in shock.

I think it has to do with the fact that we are hoping they aren't seriously hurt. That we hope it's nothing major and that they are ok. We figure if we ask the person is going to come back with it's ok and life will go on.

Usually 'cause the folks I know don't have a tendency to be physically hurt, and if they are not usually obviously so..by asking them if they're okay gives them the chance to tell me exactly why they aren't.

I think it's to show that you care. What else would you say if someone fell in front of you? It's pretty much either "Are you hurt?" or "Are you OK?"
I guess you could say "How can I help you?" but it doesn't have the same ring of normalcy that "Are you OK?" has.
