I keep holding on to someone that I really and truly love and wanna spend the rest of my life with but every time I feel like Im getting close to having my dream come true she slips right through my fingertips :(

You don't write why you think it slips through your fingers I think if you already start thinking this you send this message to the outside world as well. You should keep faith in yourself/believe that this relationship will stand and work for it without giving your partner the feeling he/she ends up in jail.

This would be probably because she is not meant for you or she is not ready to fall in love right now. I would suggest you to take your time and be patience, if she is the right person for you, she will definitely come back.
