If this bothers you then ask for the vet's advice about that so that you will know what to do the next time it happens right? Goodluck :)

Vet suggests me to castrate my dog but I don't wanna hurt him so I can only let it happens for now.

Well maybe they have some anesthesia for that? After they castrate it I think it won't be painful anymore. If you are ready then bring him to the vet.
