Problem fill a tax form in topsurfer

I join topsurfer.com . I see ptc investigation listed topsurfer as legit site. My problem is i dont know how to fill a tax form. Not only U.S resident need to fill tax form but also people outside U.S. Can you help me and give example how to fill tax form?

In some other sites like MTurk also it is necessary to fill task forms.Even though you are not a US citizen, it is necessary to fill that for.There will be provision to state that you are not a 'US citizen'. Fill and submit, that is enough.

First time i see this things. Some i not understand in there. Can you show me example of picture how to fill tax form?
