Does TV pay people for those medical realities?

I have seen on TV a patient coming to an E.R. with a sword or something like that that he got into his head.
There are many other such things.
I probably would not like anyone to film me in that condition. And neither would I want my conversations with doctors of any kind be filmed either.
So I wonder if those people who need a lot of money for their treatments (like those obese people on TV or the ones with skin problems) receive money from the TV, enough to pay for their medical problems.
I could make a long list about the new reality shows. People with physical problems, others that don´t dress well, even people in jail. I always wonder how is it that this gets on TV.

@marguicha But I saw one time that the patient came all the way from my country to go to the US to have some growth removed

Maybe there are sponsors who will pay them.

I suppose so. Because some of those medical treatments are very expensive.

I think because if they not pay then why they come on TV and tell their truth. If they do it in free then they will lose more respect and more people will know how they are and what they did.
