When Failure happens, it is a Challenge.
When Success is achieved, it is a Blessing.
When it is a Life or Death, there is a Reason or a Plan.
Just exactly, when does a Person responsible for a Failure or Success, and in Life or Death?

well, person can be responsible for his/her failure. if you don't try enough, you won't achieve what you want. person can be responsible for its success the same way.
