How did the names of the continents originate and who decided the countries which should be in a particular continent?

That has not been decided by anu particlar authority or any particular person, all these were doscovered much before by some community or group of people and they personally name those places according to their religion, and sicnce then the basic distinction in territories are being made, but yes after that more and acurate partition were made by some authorities only...
