Have you ever gone to a restaraunt and had them mess up a perfectly good meal?

My fiancee and I along with two friends of ours went out for dinner last night to TGI Fridays in Riverside California. We were seated almost immediately which for New Year's Eve Night and no reservation is almost unheard of (mind you Fridays only takes reservations for parties of eight or more.) I thought we would end up in a Seinfeld episode situation waiting for a table!
Then they moved us immediately to another table, I guess at the request of my party. I wouldn't know I was in the restroom at the time!
Finally we are all seated together and we discuss the menu. After we order our meals I picked up my silverware and noted crust on the tip of my knife. Now mind you I am not one who wants to buy a meal and get to taste another free. So I asked for another set of silverware. This prompted my table mates to examine their silverware also. All except for Rich who steadfastly refused to unwrap his until the meals came. This in spite of the fact that we had to ask for another clean place setting because there was crust of some kind on his girlfriends knife and my fiancee's fork.
Just after taking our order which was fairly basic but that in a moment. Our waitress comes over with another girl and says that she is going on a thirty minute break and in that interim our server would be the new girl. We ordered three orders of steak with 2 mashed and 1 fries. The other couple requested theirs med-well I requested mine medium rare.So we passed this time in pleasant conversation until the meals came. They passed us our plates and the couple ended up with both their steaks cooked medium rare and mine came medium with fries. so I traded steaks and plates with one of the other two as her steak was med rare but she had ordered fries and I ordered mashed potatoes. Her boyfriend took my medium well steak and I took her medium rare which left her without a plate as they took her shrimp and her fries as well back to the kitchen and left her with nothing to eat. So we dive into our plates rather than waiting for them to get cold.
My fiancee ordered the Jack Daniels ribs and was given one napkin. When the waitress came by again a few minutes later he requested more napkins after she came over and asked us, "What do you want now?"
The napkins never came until the very end of the meal and my fiancee had already gone in search of his own napkins. The other steak dinner was finally returned 15 minutes later with her origional steak which had a cut in the middle so we identified that as her same steak, except they had added another half a steak to the plate. I assume in recompense for her having to wait so long.
I must say that the food was excellent when we finally sorted out whom was getting what and dispensed with the food encrusted silverware. In fact as I am on a diet I saved a piece of my steak and two of my shrimp which everyone at the table had designs and schemes on.
And just as our checks arrived our waitress was finally back and brought us our extra napkins and wet naps for our rib eater. So then we were trying to figure out how much to tip, to whom do we tip, and what is the proper tip 15% firm? Is it based on the service that we received, or is it supposed to be 20% now?
All in all the meal was delicious, the conversation was lively and intelligent, and we had a fairly good time bagging on the service. Though it still bothers me that they had a section of a burlap bag on a shiny wooden plaqueright next to the entrance of the kitchen that was emblazoned with the name of a fertilizer company. What are they trying to say with that?
