What is the history of burgers?

I just watched the history of burgers on The History Channel. It started out in Europe more like a meatloaf and eventually bread was added. But in the USA ~ a short order cook was frying meatballs and smashed one flat with his spatula to make it cook faster. He threw it on a roll, and it took off.
